Please read the complete agreement carefully. The signer must be the athlete if age 18 or older, or the athlete’s parent/legal guardian if the athlete is under 18.
SOB Game Changers Participation Agreement, Assumption of Risk, Release, Medical Authorization & Media Consent
1. Voluntary participation. I voluntarily request permission for the athlete identified in this registration (“Participant”) to participate in tryouts, practices, training, games, tournaments, travel, conditioning, camps, clinics, meetings, and related activities organized or supported by SOB Game Changers (“SOB”).
2. Acknowledgment and assumption of risk. I understand that football, flag football, speed and agility training, conditioning, travel, and related activities involve inherent and other risks, including collisions, falls, contact with equipment or playing surfaces, heat illness, dehydration, overexertion, sprains, fractures, concussions, paralysis, communicable illness, property loss, serious injury, and death. I knowingly and voluntarily accept and assume all such risks, whether known or unknown, to the fullest extent permitted by law.
3. Health and fitness representation. I represent that Participant is physically able to participate, that I have disclosed relevant medical conditions, allergies, medications, restrictions, and emergency information, and that I will promptly notify SOB of any change. I understand SOB is not providing medical advice and may require medical clearance before participation.
4. Rules, equipment, and conduct. Participant agrees to follow all safety rules, coaching instructions, facility rules, codes of conduct, and equipment requirements. SOB may remove Participant from an activity when safety, conduct, eligibility, or program rules require it.
5. Emergency medical authorization. If I cannot be reached, I authorize SOB personnel, coaches, volunteers, event staff, and emergency responders to obtain or provide reasonable first aid, emergency transportation, evaluation, and medical treatment for Participant. I understand I am responsible for resulting charges not covered by insurance.
6. Insurance acknowledgment. I understand SOB may maintain certain insurance coverage, but no representation is made that every injury, expense, person, activity, or circumstance is covered. I remain responsible for maintaining appropriate personal medical insurance and for costs not paid by any applicable policy.
7. Release and covenant not to sue. To the fullest extent permitted by California law, I release and agree not to sue SOB Game Changers, its owners, officers, directors, employees, coaches, volunteers, contractors, sponsors, affiliates, facilities, event operators, and their respective agents and representatives (“Released Parties”) for claims arising from Participant’s involvement in the activities, including claims based on ordinary negligence. This release does not apply where prohibited by law, including claims that cannot legally be released, such as gross negligence, reckless misconduct, or intentional wrongdoing.
8. Indemnification. To the fullest extent permitted by law, I agree to defend, indemnify, and hold harmless the Released Parties from claims, liabilities, losses, or expenses arising from Participant’s conduct, rule violations, misrepresentations, or damage caused by Participant, except to the extent caused by conduct that cannot legally be waived.
9. Photo, video, name, and media authorization. I authorize SOB to photograph, record, livestream, interview, and otherwise capture Participant’s image, voice, name, likeness, statistics, and athletic performance during program activities, and to use, edit, reproduce, publish, and distribute that content for team communications, social media, websites, recruiting materials, advertising, fundraising, news, and promotional purposes without compensation. I understand content posted publicly may be copied or shared by others. I may submit a future written request to stop new promotional use, but prior publications and archival materials may remain in use.
10. Electronic records and signature. I consent to conduct this transaction electronically. I agree that printing my legal name, drawing my signature, and selecting the acceptance checkbox together constitute my electronic signature and has the same effect as a handwritten signature. I understand the system may retain the submission date and time, IP address, browser/device information, waiver version, and registration data as evidence of this agreement.
11. Governing law and severability. This agreement is governed by California law. If any provision is found unenforceable, the remaining provisions will continue to the fullest extent permitted. This agreement is intended to be as broad and inclusive as California law permits.
12. Acknowledgment. I confirm that I have carefully read this agreement, understand that I am giving up substantial legal rights, have had the opportunity to ask questions and seek independent advice, and sign voluntarily without coercion.
